Visitor Cards & Affiliate ID

 


 

 


 

  1. Select “Click Here for Service Request”
  2. Complete the form
    1. Under Common Problems select “Access – New Contractor/Visitor CCure Badges and Physical Keys”
    2. In the WO/Note Section include:
      1. Indicate you are requesting new visitor cards
      2. Include in the WO how many Visitor cards you need and if you need the U-Health Logo or Campus Logo
      3. Indicate if you would like to pick them up or mailed to you

c. Add a Chartfield for the Visitor Cards. Cost is $10 each

  1. Submit a Work Oder to renew the visitor badges click here:

  2. Select “Click Here for Service Request”
  3. Complete the form
    1. Under common Problems select “Access – CCure System – Visitor Card Renewal and Technical Support”
    2. Include Ever Card number that needs to be extended
  4. Click Submit

To avoid delays in renewing your visitor badge, we recommend submitting the request no later than the End of Day of the last Monday of the Month.

  • Vendor cards and non-university personnel affiliate IDs are for anyone who will need UCard access more than 30 consecutive days
  • Vendor cards and non-university personnel affiliate IDs expire annually

  • To request this type of UCard, submit an affiliate ID request form

Campus HR Affiliate form

Hospital HR Affiliate form

  • Affiliate forms can take up to 10 business days to process
  • Once a uNID has been assigned, the non-university personnel can purchase a UCard from UCard Services for $10.00
  • Once the UCard is obtained, a C-Cure Operator, specific to the affiliated department, will need to add the appropriate access to the UCard
  • If you are not sure who the C-Cure Operator is, submit a service request
