To receive any information on campus utilities, a valid non-disclosure agreement (NDA) must be filed with the University of Utah. More information on completing the NDA form can be found here.
Utility Locate Services (Red-Stakes)
Required Information
- Project name and project number
- Name of the University of Utah project manager or project coordinator
- If a map of utilities is requested, an approved NDA must first be filed with the University.
General Request Information
- Throughout this document, the word excavation is used as an all-encompassing term to define any ground disturbance. E.g., drilling, boring, trenching, potholing, digging and staking.
- If the excavation is due to an emergency such as a disruption to infrastructure utilities, contact the Customer Service Center immediately before excavation at 801-581-7221.
- This request does not grant the right to excavate on the University of Utah campus or properties. All excavation must comply with the project’s Building Permit.
- It is the contractor’s responsibility to contact Blue Stakes to locate all non-University utilities, including those on University property. This includes utilities such as Dominion Gas, Rocky Mountain Power, CenturyLink, and others. Contractors must ensure all non-University utilities are identified through Blue Stakes of Utah by calling 811 or visiting www.bluestakes.org.
- Requests submitted after 3 p.m. may not be processed the same day.
Red Stake details
Survey markings are valid for fourteen calendar days from the approved utility locate date specified on the form.
- Customers can request a one-time fourteen calendar day extension per survey.
- Due to unforeseen weather conditions, sites may require follow-up markings and/or flagging. This service will be provided within 5 business days of the utility request date.
- The contractor is fully liable if excavation occurs after markings are impacted due to weather or other activities.
Emergency mitigation
Facilities Management encourages excavation during the University business hours of Mon - Fri, 7 a.m. – 4 p.m., as there can be a greater threat of personal injury and property damage due to delayed response times. You are responsible for notifying the University project manager/coordinator of the excavation schedule.
Exceptions to the rule:
- Excavation is due to an emergency: immediately call the University Customer Service Center (801) 581-7221.
- Prior approval and extra precautions are in place to prevent an emergency.

Excavation Sites
Requests are only valid for the specified location per the site map submitted with this form.
- If the construction zone is fenced and the site thoroughly surveyed, the fourteen calendar days limit does not apply.
- If excavation is needed beyond the originally specified boundary, a new request is required.
- Each site requires a separate request when a project has more than one excavation site in different locations. (e.g., Lot A vs. Lot B),
- Non-fenced excavation sites must be correctly indicated and barricaded to avoid pedestrian slips, trips, and falls.
Before and During Excavation
- The university is the Utah state arboretum, which cares for over 12,000 trees. Before excavation, a tree protection plan must be in place to prevent digging or equipment storage within the protected tree zone. Please work with your project manager or coordinator to identify the protection zone.
- Should the excavation cause sidewalk impacts or closures, the contractor must follow the Facilities Management Review Committee (FMRC) approval process. The committee addresses questions of project implementation while considering the implications in terms of safety, design, accessibility, and overall fit into the campus master plan. Please work with your project manager/coordinator for FMRC review before excavation.
- The University of Utah, as a state agency, must adhere to historic preservation laws, providing cultural resource management and stewardship for significant properties in American history or architecture. Federal and State law may require active excavation monitoring by a professional archaeologist (retained by the contractor and part of the project cost) for sensitive locations.
- Subsurface containments have been located throughout areas of campus. If any evidence is encountered (e.g., stained soils, drums/containers, liquids other than water or odors), immediately cease operations and contact Occupational and Environmental Health and Safety (OEHS) at (801) 581-6590 and the University project manager/coordinator.

Utility Shutdown
Contractors, in accordance with the university Project Manager, are required to submit a Request for Shutdown Form for any and all utilities, including but not limited to the following:
- Electric power
- Low voltage
- Communication lines
- Gas
- Water
- Steam
- High-temperature water lines
- Sanitary sewers/lines
- Storm drains/lines
