It contains step-by-step guidance to make sure that your request is filled out and submitted correctly. We want to make sure you get what you need!

How to Use the New Online Requests

Facilities Management will make needed repairs in buildings or spaces. Repairs are limited to restoring what has malfunctioned back to its original condition. This does not include alterations/changes, upgrades, improvements to any facility or department owned assets.

Please visit our online Service Request for items like extra custodial services, lab equipment repair, installing a new outlet, etc.

If you are not sure if the repair you need is a maintenance item or is not in the common problems listed below, please contact your District Management team.

Corrective Maintenance Request

A Service Request is for repairs on departmental owned assets (chairs, tables, lab equipment, etc), alterations, upgrades or improvements to an existing structure or space.  Some of the services we also provide are: re-keying, key cutting, signs, name plats, metal fabrication, landscape, extra custodial services, installing whiteboards & brackets, painting, Freon removal and more. Small and large construction projects and remodels will be assigned to a Project Coordinator to oversee the work.

A verbal or email quote or estimate is not a guarantee of final price. If you would like a firm bid or guaranteed price, please fill out our online Bid Request. There is no cost to you to have a bid created.

Service Request

A Project Coordinator will be assigned to your project and they will work with you to determine the best course of action to accomplish the project. Once that has been established, they will put together a quote for labor and material and add to that total a small contingency fee to cover any unforeseen expenses. To that total a small project service fee will be added and that will be your projects final price. That total will be submitted to you for approval before any work begins. Bid requests are created at no cost to your department. If you are looking for more of a verbal type of estimate, please visit our online Service Request.

This process ensures the customer of one final price that you can budget for. These bid prices are usually only good for 30 days before work would need to begin. Bids may need to be adjusted if not approved before the 30 days are up.

**A Chartfield is required to submit a bid request. Your chartfield will not be billed unless the bid is approved. If necessary, the chartfield may be changed after this request is submitted as long as no work has been started. Please contact the Shop, District or Coordinator assigned to your request.**

Bid Request

Events are considered as large gatherings where multiple Facility resources would be involved or if assistance is needed for coordination. Events are considered but not limited to: Athletic meets and games, graduations and commencements, motocross, high school graduations, and high-profile speakers. Once this Event Request is submitted someone from our team will follow-up on details and needs.

Event Request

Our Facilities Moving Services (801) 585-6505 can accommodate your moving needs, including: furniture, boxes, lab equipment, machinery, displays, etc.

Please include the following information in your request:

  • From/To location(s) information
  • On-site contact information, if other than requestor
  • Office or lab hours where move locations will take place
  • Attach images for large lab equipment or machinery moves. If equipment is large or cumbersome, please include dimensions, weights, quantity, etc. in the description or special instruction fields. Attachments may be added when submitting a request.

Before retired refrigerated equipment can be transported, the Freon must be removed by Facilities HVAC (801) 581-8459 before Moving and Transportation is contacted. Refrigerated equipment will need to be clearly marked and correct paperwork attached.

Weather and other scheduled moves may factor into meeting your needs.
